Response rates fall with every box. Name, phone, email, city, a comment — ask for all of it and most people quietly close the tablet.

Start with rating and one tap

A score and a few tags is enough to spot a problem. Everything else is optional, and optional means the customer chooses to give it.

Ask for contact only when it pays

Phone unlocks loyalty and returning-customer lookup, so it earns its place. Email earns a thank-you. Don't collect data you won't use.

Let the owner tune it

Different businesses need different fields. A toggle that turns each field on or off — with the tablet following instantly — beats a fixed form every time.
